How to Apply

Terms and Conditions

  • We welcome applications from GPs in all four UK nations
  • To apply, please download and complete the application form and email to [email protected]
  • We will confirm receipt of your application with 5 working days. 
  • To avoid unnecessary delays, please ensure you have read all the terms and conditions and completed and checked your documents before sending

Process and Timescale

The process for GPwER accreditation is shown below. On average, the process can take about four months from submission to accreditation, if you are not required to submit further evidence.

Making an application for GPwER accreditation

  • Having checked they meet the criteria for accreditation, see Eligibility Criteria, applicants must submit their application using the pro-formas available below. Evidence provided in other formats will not be accepted.
  • Candidates must complete all relevant sections of the document to meet the accreditation requirements.
  • All GPwER accreditation submissions must be written in the English language.
  • Portfolios must contain information that relates to the candidate’s management of real patients and must accurately represent their own clinical practice. All material with patient information must be fully pseudonymised before submission.
  • All submissions must be sent by email to the GPwER Accreditation team BSACI: [email protected]
  • Only electronic (email) submissions will be accepted.
  • Material submitted in the portfolio must be current and relate to practice within a UK jurisdiction (.e. GMC regulated. If it appears that dates of evidence may have been altered (and thereby falsified), the process for “Falsification of Documentation” (Section 6.2 of these Terms and Conditions) [check] will be followed.
  • Candidate submissions will be sent to the assessors when the candidate fee and signed declarations have been received by the GPwER office.

  1. Portfolio of Evidence – Application Form  
  2. Learning Diary
  3. Patient Survey Questionnaire (for patient’ to complete)
  4. Patient Survey Data Entry (summary of patient’s feedback)
  5. Patient Survey Guidance Result Declaration (to be completed by a third party and submitted with your portfolio)
  6. Senior Clinical Supervisors Report
  7. Logbook
  8. Low Volume of Work
  9. Mitigating Circumstances and Appeals Forms

GPwER Submission Process 

  1. Submit the Portfolio of Evidence Form
  2. On receiving your onboarding letter you may commence on building your portfolio
  3. Email your completed portfolio through to [email protected]
  4. Once your paperwork is deemed in order, you will receive your invoice for payment. See our fees structure 
  5. On payment of the invoice, your paperwork is sent to the assessors
  6. Complete any interim actions or provide further evidence if required
  7. Re-assessment if applicable 
  8. Final sign off by National Lead / Steering Committee [check wording] check T&C’s note to change on assessor form.
